Identifying unmet needs is one of the most important activities in medical device innovation. Done well, it can uncover opportunities for new products and services and help determine where R&D investment should be focused.
But simply asking healthcare professionals "What are your unmet needs?" rarely produces the insight required.
The challenge is that unmet needs are not always obvious – even to the people experiencing them.
What do we mean by an unmet need?
In MedTech, an unmet need is often interpreted as a limitation of an existing product: a device is too large, a procedure takes too long, a system is difficult to use or a diagnostic test is insufficiently accurate.
These may be genuine problems, but starting with today's technologies can unnecessarily constrain innovation.
A better starting point is:
What is the healthcare professional, patient or healthcare system trying to achieve – and what prevents them from achieving it as effectively as they would like?
This shifts the focus from the product to the underlying objective, desired outcome and barriers to achieving it.
For example, a surgeon's need may not be for *a better surgical instrument*, but to complete a particular procedural step more reliably, with less variability and lower risk.
The distinction matters. Starting with the limitations of existing products tends to identify incremental improvements. Starting with what customers are fundamentally trying to achieve opens up a much broader solution space.
Why unmet needs can be difficult to identify
Healthcare professionals become extremely good at working around problems. They develop shortcuts, modify processes, combine products and tolerate additional steps. Over time, many of these compromises simply become accepted as part of clinical practice.
People are also much better at describing "what they currently do" than imagining fundamentally different ways of doing it.
Consequently, unmet-needs research needs to go beyond asking customers what products or features they would like. It needs to uncover the "friction, compromises and workarounds hidden within current practice".
How do we identify unmet needs?
There is rarely one research technique that identifies unmet needs on its own. The strongest studies combine several complementary approaches.
Map the clinical journey
Rather than discussing a device in isolation, reconstruct the entire clinical pathway or workflow.
What happens before, during and after the intervention? Who is involved? What decisions are made? Where do delays, difficulties or uncertainties occur?
Looking at the complete journey can reveal opportunities well beyond the immediate product category.
Reconstruct real cases
Hypothetical questions often generate hypothetical answers.
Instead, ask healthcare professionals to take you through specific recent cases:
"Think about the last patient where this was particularly difficult. What happened?"
Comparing routine versus difficult or successful versus unsuccessful cases can expose problems that respondents would never spontaneously identify as an unmet need.
Observe what people actually do
There is often a difference between how people describe a process and what happens in practice.
Ethnographic observation and contextual inquiry can uncover additional steps, interruptions, ergonomic problems and informal workarounds that are difficult to identify through interviews alone.
Where direct observation is impractical, video diaries, photo tasks and mobile ethnography can provide similar insight.
Look for workarounds
Workarounds are often powerful indicators of unmet need.
If healthcare professionals routinely modify a process, combine products, repeat tests, create their own tools or rely on informal systems to make something work, there is usually an underlying problem worth understanding.
One particularly useful question is:
"What do you do today that you feel you shouldn't have to do?"
Explore edge cases
The clearest unmet needs are not always found in typical patients or procedures.
Complex patients, difficult procedures, resource-constrained settings, inexperienced users or cases where treatment fails can amplify problems that exist more subtly across the wider market.
Challenge existing assumptions
Stimulus can also help respondents think beyond today's technology – without asking them to evaluate a specific product concept.
Questions such as "What if this step could be eliminated completely?" or "What would change if you had this information 24 hours earlier?" can reveal needs that the current technology environment may be masking.
From observations to clearly defined needs
Identifying problems is only the beginning. The research must translate individual observations into clearly defined underlying needs.
Crucially, these should be expressed independently of potential solutions.
Instead of:
"I need a smaller device."
the underlying need might be:
"I need to deliver the therapy without restricting the patient's mobility."
This keeps the solution space open. A smaller device might solve the problem – but so might a wearable system, a different treatment approach or eliminating the device entirely.
Which unmet needs are worth solving?
Not every unmet need represents an attractive innovation opportunity.
Once needs have been identified, quantitative research can determine their relative importance and the extent to which they are currently satisfied.
The most interesting opportunities typically sit where there is:
**High importance + low current satisfaction**
Techniques such as importance–satisfaction analysis, MaxDiff and Best-Worst Scaling can help prioritise needs. These can then be considered alongside factors such as the number of patients affected, impact on clinical outcomes and workflow, willingness to change, competitive activity, technical feasibility and commercial potential.
The result is not simply a list of customer complaints. It is a **prioritised opportunity landscape that can help guide R&D and innovation strategy.**
From unmet need to innovation opportunity
The purpose of unmet-needs research is not to ask customers to design the next medical device.
It is to understand their world deeply enough to identify problems worth solving.
The most valuable opportunities are often hidden in everyday frustrations, compromises and workarounds that customers have simply learned to accept. Finding them requires looking beyond today's products and understanding what clinicians, patients and healthcare systems are fundamentally trying to achieve.
